  • Commit to using your ElationHealth.com Portal

    • All communications with our practice will be through your Patient Passport, which is HIPAA compliant.

    • Patients are discouraged from using their Passport to message Dr. Salt more than 2 times in between appointments. Please be considerate of his time and do your best to keep your messages to him short!

Late Cancellations & “No Shows”

When you schedule an Initial Consultation, Dr. Salt does an extensive review of your Intake Form and medical records prior to your appointment. Additionally, we are holding a one-hour time slot for you.

If you need to cancel your appointment, it our practice policy that you do so no later than noon on the Friday prior to your appointment. Cancellations after 12 noon on the Friday prior to your appointment and “no shows” will be billed $300 for Dr. Salt’s review of your Intake Form and medical records, and also for the lost one-hour revenue.

We promise to work hard to help you achieve your gut health solutions.

We promise you unfailing personal time, friendliness, and attention to your health details.

When you make the decision to become our patient, we immediately enter into a relationship with you. We make promises and so do you.

Your most important promise is to diligently partner with Dr. Salt.

Dr. Salt frequently orders tests (such as labs, breath tests, etc.). Your test results are pieces to your unique puzzle. Dr. Salt relies on your promise to complete these as soon as you can! Dr. Salt is highly skilled and able to help you but it is important to remember that you are half of the partnership!
